These are images of a Behavior Support Plan of a Re-Evaluation IEP that was held on the last week of school. The student is assigned a BII and BID from a non-public agency to address his behavior. He exhibits tantrums and non-compliance and it is impeding his learning because they result in outbursts/rage/explosive reactions. The student currently receives Occupational Therapy and ERICS Counseling services. Parent, administration, teacher, and service providers called for a re-evaluation to determine a more appropriate placement due to the student making little to no progress in regards to his behavior. A Functional Behavior Assessment was conducted to gather data on the student's problem behaviors. The decision was made by the team to change placement from Special Day Class to Non Public School due to the severity of the student's behaviors.
